New Patient Registration
Here are the forms needed at your first appointment at DESERT HEALTH CLINIC. Print them out, fill them out and bring them with you to your appointment.
New patients: Please arrive 30 minutes prior to your first appointment time in order to ensure all Patient Registration is in order.

Fees / Cancellations
Psychiatric Service:
Initial Diagnostic Evaluation - 60 min – $325
Follow Up Appointments - 15 min – $150
Psychological Service:
Counseling Services Provided By Independently Licensed Psychologist, Social Worker or Family Therapist: Psychotherapy 50 min- $140, Psychotherapy 75 min- $210
Counseling Services Provided By Associate Licensed Therapist: Psychotherapy 50 min- $110, Psychotherapy 75 min- $165
Counseling Services Provided By Supervised Psychology Extern: Psychotherapy 50 min- $50, Psychotherapy 75 min- $75
Life Coaching - Self Pay (Individual $80, Couple/Group $100)
Massage Therapy - Self Pay and HSA (health savings accounts)
40 minutes $79, 70 minutes $139, 100 minutes $199
Cancellation Policy:
24 business hour notice for current patients.
48 business hour notice for new patients.
If you cancel a psychiatric service appointment, there is a $50 fee for 15 min appointments. If you cancel a psychological service appointment, the fee is the same as the corresponding session’s fee.
